2 items: Casting an ability that costs Health spawns a blood ball for 10 seconds. This effect can occur once every 10 seconds. You or an ally near the blood ball can activate the Sanguine Burst synergy, dealing 2879 Disease damage to nearby enemies and applying Major Maim for 8 seconds, decreasing their damage done by 10%. Enemies can only receive Major Maim from Sanguine Burst once every 20 seconds. The damage scales off the synergy activator’s Max Health.

This is a Tank monster set that can be used in trial add pulls.
You need to use a Health costing ability such as Balance or Blood Altar to creates a self-synergy which you can create every 10 seconds and use every 20 seconds. When you use the synergy, it applies a debuff to all enemies 360 degrees of the blood ball that it spawns causing AoE Major Maim. The Major Maim can be stacked with Minor Maim to debuff an enemy’s damage by a total of 15%.
This set is still ok for debuffing hard add pulls but since Tank’s don’t use Alkosh anymore and we have access to Major Maim with a 100% uptime via Frost Clench, this has really diminished it’s value.
